Compounding pharmacies have been linked to deaths, illnesses for years

When 45 people died after receiving tainted steroid injections from a Massachusetts specialty pharmacy, industry officials distanced themselves, saying the pharmacy was a rogue and an outlier.

But a Washington Post analysis of court and government records shows that three other large-scale compounding pharmacies had problems long before last fall's meningitis outbreak linked to the contaminated steroids.

Shoddy practices and unsanitary conditions at the firms were tied to at least six deaths and 39 illnesses. The problems included medications that were too potent or laced with bacteria.
